HOW DO YOU BEGIN?
As a participant in the Self-Determination Pilot
Project, here is what you would do.
- Form a circle of support. (A circle of support is
made up of people you encounter in your
everyday life that you trust
and can depend on to help you.)
- With the help of your Circle, develop a person
centered plan that tells what you want to do.
- Identify the supports and services you need.
- Create an individualized budget to pay for your
supports and services.

HELP IS AVAILABLE!-Start up Brokers
Funds are available to pay for someone to help you with the application process. These people are called "Start-up Brokers."
A directory of start-up brokers who are located in your area can be obtained by:
- Contacting your local OMRDD district office (DDSO) - see below for telephone numbers,
- Visiting the NYS Self-Advocacy website at www.sanys.org and clicking on the Start-Up Broker button, or
- Calling OMRDD at 518/474-5647.
